During a time of family crisis you need a place to begin gathering information. Often, in times such as this, you don’t even know what it is you need. Below are some helpful links to local organizations that will help you get started in a time of crisis. You’re not alone – there are people and organizations out there ready and trained to help.

We’ve also included a link to our directory, some information you’ll need when helping your loved one, and a checklist provided by AARP in their Prepare to Care – A planning guide for Families that will help you to organize your information.

Tri-County Office on Aging

Their website will get you in touch with their office, useful information regarding local resources and links regarding Medicaid eligibility and Medicare resources. This is a great jumping point when you really aren’t sure what you need. Give them a call at 517.887.1440

Alzheimer’s Association

Their website will connect you to resources regarding dementia and Alzheimer’s disease and can help you to find local support groups. You can also check out our directory for links to local support groups.

State of Michigan Office of Services to the Aging (OSA)

Here you can find helpful links to senior services throughout the state.

For information on and directories of hospice organizations throughout the state

Elder Law of Michigan

They provide services that address a variety of issues and problems – including legal, financial, pension, economic, food security, and more.

Referral for safe driving form from the State of Michigan (DA-88)

An anonymous form to have a senior come in for a driving test.

Senior Driving information

Quick list of things that may be needed during a time of crisis:

  • Durable Power of Attorney
  • Living Will
  • Do Not Resuscitate Order
  • General or Financial Power of Attorney
  • List of all medications
  • Health insurance cards and numbers
  • Wishes about Funeral, Burial
  • Copy of Will
  • List of important people, family, attorney, doctors, financial or other advisors, etc
  • List of financial assets and other assets – location, form and value
  • List of insurance policies, values and agent
  • Information on retirement plans and contact person
  • If past employer is still providing benefits, life and/or retirement funds, name of employer and contact person
  • Birth certificate, marriage certificate, military discharge papers
